The tutorial at /products/xmg_faq.html#d-3 does not cover Kubuntu v6.10.
I have edited /etc/kde3/kdm/kdrmc to include
Enable=true
Xaccess=/etc/kde3/kdm/Xaccess
Willing=/etc/kde3/kdm/Xwilling
I restarted kdm but when I run Xbrowser, I get
*Waiting for client connections ...
Xdmcp fatal error: Manager unwilling Host unwilling*
Xstart's SSH connection works fine.

Have you done this?
Open /etc/kde3/kdm/Xaccess and remove the initial ¿#¿ character for the following line:
#* #any host can get a login window ==>
* #any host can get a login window
